A Clinical Study of Total Gastrectomy for Gastric Cancer



Abstract
A retrospective study of 125 patients with gastric cancer treated with total gastrectomy performed between Jan. 1988 to Dec 1992 at Dept. of Surgery, Kwangju Christian Hopital and analysis them.
@ES The results as follows:
@EN 1) The most common age group was 60's (36%), and male (87) to female (38) ratio was about 2.4:1.
2) The common clinical symptoms were epigastric pain and discomfort, indigestion, nausea and vomiting, weignt loss etc.
3) The most common site of tumor was upper body of the stomach, 71 cases (56.8%), and predominent histologic type of the cancer was poorly differentiated tubular adenocarcinoma in 86 cases (68.8%).
4) The radical operation was done in 78 cases (62.4%), and palliative operation in 47 cases (37.6%), and splenectomy was also done in 37 cases (29.6%), and extended total gastrectomy in 13 cases (10.4%).
5) The methods used for postoperative intestinal reconstruction are loop esopnago-jejunostomy and Braun anaaaastomosis in 21 cases (16.8%). Roux-en-Y esophagojejunostomy in 82 cases (65.6%), ¥ñ-method in 9 cases (7.2%), and afferent jejunal
ligation
and side-to-side jejunojejunostomy in 13 cases (10.4%).
6) The types of intestinal tubes used for postoperative intestinal decompression and enteral hyperalimentation were 20Fr. Levin tube in 44 cases (35.2%) and 6.5Fr. ED tube in 81 (64.8%) and there was no differences in postoperative drainage
amount, but
complications were more common in case of 20Fr. Levin tube was used.
7) Postoperative complications were wound infection in 12 cases (9.5%), pulmonary complications in 7 cases (5.6%), and operative motality was 3.2%. Cause of death were leakage of anastomosis site in 2 cases, hepatic coma in 1 cases, and shock
due
to
bleeding in 1 case.
8) The one year and two year survival rate were 78.3% (65/83) and 60.8% (45/74) respectively.
